Problem:

The circuit breaker TIB was sent by mistake on two dates, November 23, 1994 and again on November 30, 1994.

Solution:

Destroy the November 30, 1994 TIB. It will not appear in the Quarterly Index.

Destroy this TIB. It will not appear in the Quarterly Index.
