Caterpillar has had an ongoing program for many years to obtain written incident reports involving Caterpillar products. The majority of the incident reports are filed by Caterpillar Dealers. This system has been very effective and we encourage continued participation in the reporting process. The written system will continue to be available as it has in the past. However, Caterpillar has developed a new faster means of reporting incidents to Caterpillar. If you have access to the Caterpillar Intranet, you will be able to submit an incident report electronically. By logging into the Cat Neutral Net, an electronic incident report can be filled out, printed, and submitted. BACKGROUND INFORMATION ON INCIDENT REPORTING

Any incident is an event involving any Caterpillar product which results in one of the following: personal injury, death, product damage, property damage, or an especially unique event though no injury or damage occurred. The purpose of the report is to gather and record objective factual information concerning the incident which will allow Caterpillar to evaluate the need for product changes. Information regarding the incident should be reported as soon as possible after the incident occurs. This will enable Caterpillar to quickly conduct further investigation and follow-up as required. Additional information can be sent in later. Accurate and factual information is essential, since incident reports may subsequently be reviewed in the course of future lawsuits.

WRITTEN INCIDENT REPORTING SYSTEM

Caterpillar supplies a prepared form to make reporting of incidents easier, more uniform, and more accurate. Attached is an updated Incident Report Form that replaces the prior form. Additional forms may be duplicated from the one provided, or you may order pads of 50 through the regular Literature Order System. Request Form No. 01-082918-07. Complete instructions are on the back of the form to explain how to fill out and submit the report.
