NOTE: | This Containment Program can be administered either before or after a failure occurs. In either case the decision whether to apply the Program is made by the dealer and service rep. Obtain the service reps Goodwill Approver Code before completing the repair. EAME dealers should use their standard Industry Group specific goodwill approval codes. The Goodwill Approver Code must be included when submitting the claim. |
NOTE: | When reporting the repair, use "PS90522" as the Part Number and "7777" as the Group Number. If administered before failure, use "56" as Warranty Claim Description and "T" as the SIMS Description Code. If administered after failure, use "96" as Warranty Claim Description Code and use "Z" as the SIMS Description Code. |

PROBLEM
There have been reports of 8W-8739 (RH) and 8W-8741 (LH) Axle Shafts failing, in certain applications, on certain 789C Off-Highway Trucks. These failures can lead to unplanned machine downtime and resultant component damages. |

ACTION REQUIRED
First identify the two types of shaft as per below image on the face of the shaft. If the truck has a shaft similar to the image on right side do not replace. If the shaft is similar to the Left side of the image then look for the date code below 1205xxxx and replace it with the new shaft. If the shaft has a date codes above 1205xxxx do not replace. Document date code identified on the shaft when inspection is done and add it in claim report. |

If there has been a previous repair, part age/hours will apply. Retain a copy of the previous repair invoice in the dealer's records for audit purposes, and specify repair date and machine hours in the "Additional Comments" section of the warranty claim. |

Note 1: Total of 4 hours is required for date code inspection on both sides. Note 2: Before failure, an additional 2 Hours is required for shaft replacement on each side, based on the date code. Note 3: After failure, an additional 50 hours of labor is required for additional inspecting of axle group and flushing the system. |
