Faulty Fuel Injection Nozzle Adapter
Check the fuel injection nozzle adapter for damage. Ensure that the gasket and the seal are not leaking. Check the tightness of the fuel injection nozzle adapter. Refer to Disassembly and Assembly, "3406C Truck Engine".
Check the cylinder head gasket for leakage. Refer to Troubleshooting, "Engine Oil in Cooling System".
